I found just about anything tolerable, as long as the bed that I was sleeping in wasnt crawling with filth. Although I thought Bluefields was a very quaint and interesting place, it was probably the most unhealthy place I visited, and I spent quite a bit of time way out in the campo of north Nica. Little Corn was a bit cleaner in the gringo sense, most likely do to the increasing tourism. Most folks merely land on Big then take that notoriously rough boat ride you were talking about. It was all I could do to keep my lunch down. For those planning to stay awhile, most whities would find Little Corn quite a bit more comfortable than Big.
